Our adult day care boasts equipment and features that allow us to provide specialized services for seniors.

Medication Management

Medication Management is available to all individuals that attend Senior Community Outreach. Medication prescribed by a physician will be administered by a licensed nurse at the prescribed time and ordered dosage. You no longer have to worry if your loved one is taking and receiving important medications. The nurses at Senior Community Outreach are here to partner with the physician to ensure success with this important task.

Caregiver Education & Support

At Senior Community Outreach we understand that caregiving is demanding and that you face special challenges. You are not alone. We provide personal insights and valuable resources. Whether you need help with daily living, safety planning, or community support, we are here to assist.

Therapeutic Recreational Activities Conducted in English & Vietnamese

Our therapeutic recreational activity program is under the direction of an Activity Coordinator. Each activity can be modified and is designed to match the personal abilities and needs of all members with an emphasis on cognitive and sensory stimulation. The music and aromatherapy enjoyed by members helps to reduce behaviors like agitation and wandering. The daily participation in physical exercise increases strength and stamina. The brain health activities like trivia, brainteasers and reminiscing increase cognition.

HYDRATION PROGRAM

The hydration program at Senior Community Outreach includes making small, frequent amounts of fluids available during and in between all meals and snacks. Our health care workers play an important role in keeping our members properly hydrated. A licensed nurse assesses all members for their risk

of dehydration. The dietician plans meals to increase hydration by including fresh fruits and vegetables rich in fluids. The Activity Coordinator often plans events like a special meal or an afternoon tea time to promote healthy hydration and socialization. Care attendants ensure scheduled bathroom breaks are conducted to alleviate any fears of an “accident” that members may have.

INDIVIDUAL PLANS OF CARE

Prior to admission at Senior Community Outreach the facility nurse conducts a nursing assessment with each potential member and their family. Additionally, the primary care physician completes a physician’s assessment. The information received from the assessments along with input from family is used to create an

Individual Plan of Care. The Individual Plan of Care is essentially a master plan that outlines how a member’s care will be implemented. The Individual Plan of Care is an ever-changing document that is routinely revisited by physicians, nurses, direct care staff and family. Changes are made to the care plan as a member’s condition improves or worsens. Having a plan in writing is beneficial for everyone involved. Family members can feel more comfortable knowing exactly what kind of care their loved one will receive. Members are assured higher quality, consistent care to meet their changing needs and direct care staff has clearly defined roles. In short, everyone is on the same page working to achieve the same goals and objectives.

NUTRITIOUS, MEALS & SNACKS

At Senior Community Outreach we know that food and nutritional needs change, as a person gets older. Older adults generally do not feel as hungry as the early years. Under the direction of a qualified staff member we have put together a variety of nutritious, satisfying meals and snacks. Each day breakfast, lunch, and afternoon

snacks at Senior Community Outreach are happy times with a chance to socialize and talk with others in a clean environment.
